I have a theory that this album art is part of a big promotion for the 'Judas' single focusing on her tweet "Even After Three Times He Betrays Me." I think she's going to 'betray' her fans three times, the first perhaps being the early release of 'Judas' and then the album artwork. (which is too ridiculous for me to ever take seriously) The third betrayal is yet to come... The 2nd 'Born This Way' video perhaps? Mightn't even exist? Maybe that's a 'betrayal' that's already taken place, I don't know. Take me as a super fan - but it'd fit in well with the single! (wishful thinking anyway)
